How one should conduct incident /accident witness interview? What one should emphasize during incident /accident witness interview?


Incident /accident witness is key concerned to ascertain the cause of the accident/incident because he was the one who might have seen all the sequence of the incident/ accident. So it is very crucial to conduct incident/ accident witness interview in proper manner. 






There are some guidelines to follow which are as mentioned below:
1-      Conduct interview in a closed and silent room where both interviewer and witness can hear each other correctly.
2-      Voice should be as much as low possible, this will keep witness at ease.
3-      Conduct interview of one witness at a time.
4-      Calm down the upset or angry or emotion full witness. This will serve two purposes; witness get ease and also he/she can remember the sequence of events easily.
5-      Give full time to witness to recall the events sequence. This will help you to ascertain the maximum evidence.
6-      Try to start with open ending questions and in last ask close ended questions. Open ended question; what was happen? Close ended question: Which machine hits his hand?
7-      Focus on finding facts rather than focusing on fault? i.e. Why accident happened? Do not focus on who was responsible?
8-      Do not interrupt witness in-between; let him talk till the end. This may lead to forget witness important fact.
9-      During interview try to make notes and these notes should be of short notes type.
10-   Recheck noted statement with witness and take his consent about your notes.
11-   Confirm with witness about any other aspects he wants to explain related to accident/incident.
12-   Never intimidate the witness. This will destroy the whole purpose of investigation and also impact the future investigations, belief on organisation.
13-   Take time, pause in between the questions. This will help to avoid ambiguity, improved understanding of the witness about the asked question. Never ask questions in hurry.
14-   Never show your own emotions. Always try to find facts, never show any relationship, friendship etc while doing interview.

There are some common questions which should be asked while conducting interview. These are as below:

1-      What is your name?
2-      What is your Job?
3-      What were you doing at the time of interview?
4-      What did you see?
5-      What did you hear?
6-      How did injured worker collide with the machine?
7-      What is your thought about the cause of the accident?
8-      How you can prevent similar accident in future?


Always greet the witness in last, tell him not to disclose the interview session with anyone, always keep organisation first and dictate him about the purpose of the incident investigation once again .i.e. Fact finding not fault finding.
