What is accident? What are the benefits of Accident investigation? What are the common errors we do post accident?  Is accident investigation an active monitoring or reactive monitoring?

Unexpected and undesired event lead to injury or illness to an individual is an accident. An event that results into injury or ill health (Provided in HSG245). At workplace while performing task by an individual either due to his own unsafe act or by others or due to existed unsafe condition undesired outcome is an accident. Also energy from the process equipment like conveyor belts, pulleys, vessels etc. flows in wrong direction and interacts with human (workplace worker etc) results into accident and ultimately injury.


Post accident errors done by organisation :
-          Forget to report or hide the accident leads to probability of recurrence.

-          Manipulation of the site condition results into wrong judgment about the cause.

-          Blame game initiation results into facts hiding by the concerned. i.e. worker, witness.

-          Late investigation probably results into loss of many significant facts.

-          No communication of the accident as well as root cause findings or learning of the accident.

-          Incompetent personnel in investigation team like absence of concerned skill. i.e. like doing investigation of electrical substation without involving electrical expertise.

Similar many other errors we do and we should avoid these errors.

Benefits of accident investigation in true sense:

-          Identification of root causes results into reduced probability of the similar accident.

-          Legal compliance with respect to accident investigation and causes and control measures should be included in risk management process in case it is absent.

-          Reflection of management commitment. Doing investigation and implementation of its findings to prevent recurrence reflects management commitment.

-          Improved morale of the individuals like workers, bottom and middle management worker.

-          Process continuity without disturbance due to accident.

-          Availability of skill (worker) that got injured during accident.

-          Workplace unsafe conditions elimination in case accident happened due to that unsafe condition.

-          During investigation other unsafe conditions or unsafe practices visualize and which results into prevention of other accidents.

-          Helps in identifying breach of duty of care.

Many more other benefits existed for doing investigation and its learning implementation.

Accident investigation is a reactive monitoring tool which we do to prevent future accidents. Always it is better to perform active monitoring. Here organisation can focus on hazard reporting, its correction and safety observation tour. Hazard reporting will help in reduction in unsafe condition and safety observation tour will help in reduction of unsafe practices. So doing both ultimately reduction in accident will happen because unsafe practices and unsafe conditions are the foundation to any accident.
